Direct service provider

Job Description

The qualified candidate will provide community & in home support and services to consumers with developmental disabilities.


The primary goal of ILS is to provide community based support. This will emphasize improvement in the quality of life through maximizing independence, community inclusion and personal growth. 


The training is to provide skills which will assist the consumers to live independently in the community and to participate in society. In the short term, consumers learn task in a curriculum arranged from simple to more difficult tasks. Specific goals are determined periodically by the Vendor and the Regional Center staff in coordination with the consumer and (where appropriate) the consumer’s family.


Duties include but are not limited to: 

  • Cooking & Meal Prep
  • Cleaning and Laundry Skills
  • Money Management
  • Meal Planning
  • Self-Advocacy 
  • Community Safety
  • Household Management
  • Assist with appointments:
    • Doctor 
    • Dental
    • Specialist
    • Social services 
  • Provide daily progress notes.
  • Other related duties

Qualifications


  • Minimum of 18 years of age.
  • A demonstrated ability, desire and dedication to work with people with developmental disabilities.
  • Working knowledge of Social Security, Social Services and Medicare helpful but will train.
  • Must possess excellent interpersonal skills and communicate effectively verbally and in writing at the highest professional level with agency staff, consumers and community members.
  • Must be proficient in the use of a iPad or computer, including Google drive, Microsoft Office 2010 products: Word, Excel and Outlook. Ability to work independently to produce all correspondence, record keeping and related documents.
  • Ability to maintain confidentiality and composure when dealing with highly sensitive, confidential matters as required.
  • Infant, Child & Adult CPR and First Aid Certified.
  • Must have cell phone  
  • Must have a driver’s license and have a vehicle to drive.
  • Part-Time/ Full Time Hours 
  • Some requirements may be waived because of previous experience.

Performance Criteria

 

  • Development of strong working relationships with all relevant organizations and staff
  • Enhancement of the agency’s image 
  • Successfully carrying out the prime functions of the job description
